allow provider to add addendum withing notes application that also allows them to sign addendum and cosign

Are providers that add an addendum to mid-levels notes, has to do it from the task list as if they add addendum in notes, their is not option to sign addendum AND cosign, so even thought they added an addendum, their name does no show at bottom of...
