Need ability to change headers on Note documents Merged

We are looking to be able to create a Note to go to the patient's portal like a work or school excuse note which can be sent to the patients portal electronically without having staff scan it in. Notes is not an option as it includes the patient't demographics and it is not always necessary to have this information on the excuse form.
