Flow chart that can be opened for the same patient on multiple accounts

Our outpatient infusion/injection nurse would like a flow chart in Evident that can follow a patient and be "reopened" on future account numbers. The idea is to be able to track and continue documenting on those patients that have weekly or monthly infusions or injections and see what was done last time instead of having to open the previous chart. This would also likely be used by clinic nurses who are following patient INRs.
