Text boxes next to Isolation check box to identify the why

I think staff want to know what organism the patient has. For instance, airborne isolation due to Covid or tuberculosis, etc. Especially staff that need to enter the room but are not taking care of the patient. I can't answer to where it should pull to unless someone would want to build an Ad hoc report on the patients on isolation and why they were on it.

Could I get some more information on what type of information would be added in these text boxes? I am thinking of the length needed for the text boxes. Where would the information entered in the text boxes pull to?
