HIM: Split out HIM Deficiencies and Queries into it own Application Merged

Application Code: Physician Documentation is needed in order to use Physician Query/Def Note. The ask is to simply split off the HIM deficiencies and queries functionality into it's own application that can be activated/deactivate separately from the base PhysDoc application.
