Provider Address in Patient Schedule

Address for the Provider does not show up in PDF of Patient Appointments. It would be nice to have that information available to patients when you hand them a list of appointments. It only shows up for "Locations" but not for individual providers. Also, would like the patient's address to show at the top of that form for mailing purposes.
