Right now the only options to route orders and/or notes to supervising physicians is either to have the supervising physician sign all the orders/notes that are done by the midlevels that need a co-signature, exempt co-signature on a specific note type, or have midlevels choose which notes/orders get routed to a supervising physician. I think that we should be able to assign a % of notes/orders that are assigned to a supervising physician via stay type/service code and/or per supervising physician. Each facility probably has a different percentage of required reviews for different stay types/service codes and maybe even for each supervising physician, and we should be able to determine that with a percentage in the system that automatically distributes the notes/orders according to that percentage. Then there should also be a report for those co-signatured items that shows the stay type, service code, supervising physician, and midlevel associated with the episode so that it is easily tracked in the system.
