When adding existing home medications, there is currently not a field to enter duration. There are times when patients should only take a medication for a certain length of time (example: oral antibiotics). This will notify the physician how much longer they are on that medication. Thrive suggested using the comments section, which is not ideal as the comments do not cross over when completing an admission medication reconciliation.
We actually just had this issue come up not long ago. After testing, we found that the comments do cross over during admission med rec. We have pointed this out to the providers. The bigger issue is that they can't take action on it while going through each med. We now ask that they review the comments in the pending order queue and then modify or add orders as needed to address if patients are on a taper, have stop dates, etc. I do think it would be nice if there was a way to take action while going through each med on the admission rather than having to wait until the end. I logged in as a test physician and recorded the process to point out to them what to look for.
