RX Entry without dc Med Rec poses med error risks with no safeguards

Awhile ago TruBridge changed the functionality of med rec to allow physicians to escribe meds before a discharge med rec was performed. This change was helpful for a select few cases but poses some medication error risks. This change allows nursing access to prescription entry without a discharge med rec being performed- previously it did not allow them to enter and would tell them a discharge hadn't yet been performed. We recently had an incident at our facility when the nurse went into RX entry to enter next dose/date times not realizing that a d/c med rec had not been performed for this patients visit and entered info onto old Rx's (that were no longer prescribed for this patient) that would have subsequently printed on the patients dc instructions. The system needs some kind of alert or roadblock so that a patient isn't sent home without having a dc med rec performed. This is a patient safety issue if they are being sent home with old dc instructions from a previous visit.
