Report locations advancement

Would like to see increased functionality in the report locations table. Need more felids for that table to include more than 15 characters for the description. Allow to add an address and phone number to the location. Hard to use this table in conjunction with other departments.

Our facility is attempting to use report locations for our provider table and the limited functionality makes this difficult.
