Add MEWS Scoring tool to Vital Signs Selection

Currently we have this in a Flow Chart but it is difficult to view the vitals and use the scoring tool that way. We are also trying to move away from Flow Charts and this would make it feasible. This scoring is used in many facilities and I'm sure would be appreciated.
