Flowchart - Embed most common pain scale options

I would like to be able to click a Pain Scale Question in the flowchart and have it pull open a pop up screen that has options to pick from multiple common scales like Numeric, Verbal, FACES/Wong-Baker, FLACC, VAS. Once you select your scale, it would then open up that assessment and you could document that assessment and then the Answer would indicate the score in the flowsheet answer box. (Additionally, it would be really nice if this could be tied in to PRN pain med administrations and prompt a pop up within the MAR that would then place the answer back into the flowsheet as well)
