Add Authorized Rep to Demographic Banner

It is urgent that there needs to be a better place to identify an authorized rep. People need to know who they can and can't speak to right as they receive a phone call about a patient. This is not even available on the standard Facesheet which is what most people utilize for contact information and also when sending referrals out or on System Menu side where it is easily accessible. Several clicks have to be done to even find this. Many mistakes are made because of this.

Whether this is added to the Demographic Banner or the Facesheet, it is urgent that this be somewhere that is easily and quickly accessible.
