ED Discharge/Disposition/transfers ER log dispo crossover to Location Maintenance.

Good Morning,

Our ER, must document 3 places for patient dispo/transfers, and discharges in order to pull to ED report....As of now, the ER log does have a place to document the discharge and/or transfers, but does not cross over to Location Maintenance, however Location Maintenance documentation does cross to the ER Log. Also, the disposition documentation could be missed because there is not a hard stop prompting the nurse to chart.

Our Request is to have one place to document dispo/transfers, and discharges: to Ensure pertinent documentation will not be missed and to streamline ED charting. Screen shots are provided with current workflow for removing the patient out of the system. We appreciate your help.
