Add custom disallowed passwords and expand default options

Allow sites to add keywords, such as their facility name, to a list of non-usable passwords and/or the dictionary. Expanded options, such as requiring the default site password to be changed, would also enhance security hygiene at many locations, as many default passwords tend to linger between different systems and software.
