Layman's Terms, Step by Step guide for Quality Measures

It would be nice if there was a step by step guide, in layman's terms, taking a starting from nothing approach to Quality Measures. Showing us exact setup, exactly where and what to document, what not to document, how documentation flags a patient for the quality software - in a way that someone, who has no experience in quality and may not be super comfortable with the software, can learn from the ground up what we are supposed to be doing. From the reporting side, to the registration, nursing, coding, etc.
