Streamlines Swallow Evaluation for Emergency Department

Could we have a multi-clinician document created for a simple swallow screen for our Emergency Department. The one that is currently in the system is not the most current recommended swallow exam. It is too in depth and involves having patient swallow apple sauce, then juice, etc when current nursing screenings only involve having the patient swallow approximately 3 ounces of water without choking. I am attaching a flowchart that lays out how the document should look. I attempted to edit the one that is currently in the system but do not have access to do so. This would streamline the process for our emergency department.
