Add Authorized Reps to the printable Facesheet.

Most of the time when doing callbacks to patients, the Facesheet is a good option to look at phone numbers. It would be easiest to view the patient's authorized reps of who we can speak with by giving the names on this sheet since this is what the clinical staff looks at when calling back labs, referrals, etc.

This also needs to be added in System Menu side under Contact/Billing Info for easy readability. Many times, people are mistaking an emergency contact for an authorized rep when they are not the same. The "NOTIFY" under Contact/Billing on System Menu is confusing. This needs to say in case of emergency. There needs to be an Authorized Rep place beside this stating that this is who to contact to relay information.

On new patient paperwork, we provide two spaces for patients to enter two authorized reps when the CPSI system will only allow one to be entered. Can we allow two Authorized Reps in the system?
