Our OB department manager would like more of the fields required or the option to require with a answer or N/A as many of the fields are being missed, specifically:
VFC status
Vaccine Funding source
Publicly supplied (check box)
Vaccine information statement reviewed (checkbox and date)
Vaccine information statement version date
We participate in the VFC program and details such as those mentioned need to be assessed at every visit as required by DSHS. It should be an option for us to put a hard stop for those fields to be answered because not all facilities participate in VFC program.
