Update the labels from MAR to pull provider info from Provider tables

The current EMAR label and UDEF pharm labels are hard coded with "DR." in front of the prescribing/attending providers name. Not all providers are DR... would like this removed/updated. It makes more sense to pull their title from the physician table set up... APRN, PA, MD, DO...
