Add DECLINED to RELATIONSHIP LOOKUP CODE

Can DECLINED be added to the Relationship code as there are instances where patient refuses to provide any but it is not UNKNOWN to patient. SIT#10222529 request for this was declined as it requires a SW change. (Not Table driven)

we do love the new auto create feature but when you accidently create a new visit there should be an option to merge the two. if i can move a visit form one pt to another patient made in error i should be able to merge two visits into one.
