Add appointment time to referral/transition of care form next to appointment date field

So that patient can see both appointment date and time in same area without having to search other places in patient discharge instruction paperwork. It will be helpful to have this next to the appointment date field instead of having to free text...

Would like to add patient screenings for sepsis, covid, suicide, etc., that can be required assessments to be documented for nurses. Health Hx application currently asks the Depression assessment.
