Patient Connect Opt In Default

In Patient connect would like a feature to opt in all patients automatically. When you push them the opt in opt out policy, if they opt out, it will opt them out. Otherwise everyone is opted in.

Ability to type notes on the Scheduling side or to show System Menu notes when scheduling the patient so if a patient is "discharged/fired" from our care, we know not to schedule the patient.

We need the ability to type notes on Scheduling side or allow System Menu notes to appear when scheduling a patient. This is because if a patient is discharged/fired from our care, we need to know to not put them on the schedule again if they call.
