Editing existing flowcharts for a must answer condition

How do I make some fields in a flowchart a "must answer" before saving the entire flowchart. We have a pain assessment flowchart on admission where we have to ask the acceptable level of pain to patients that we must answer for a JCAHO requirement.

Problem list in flow chart: if the title is too long that the field elongates to allow a second line

If a problem is put into the flow chart that is 50 characters long but the field only shows 40 characters on that screen size without it going to a second line, it puts the rest of the characters on a second line, but the field does not get longer...
