Hover for comments on med rec

Would be very beneficial to be able to hover over a med in the medication reconciliation and be able to see a comment if one is there. Currently we have the comment bubble indicator but you have to click into the med to see what it is.

Prevent deletion of previous users communications

If someone is replying within the communications tab, they have access to delete the prior conversation. This prevents continuity of care and what has already been completed or discussed between staff, the provider and the patient.
